This NP position creates a Medicare pathway making life-changing dementia care accessible to families who couldn't otherwise afford it.

Why This Role is Exceptional As the Nurse Practitioner leading the GUIDE Program, you'll provide comprehensive dementia care while receiving world-class mentorship. You'll work directly with Dr. Sandison and receive 3 hours of weekly training from Dr. Dale Bredesen, the physician who created the ReCODE Protocol and pioneered the functional medicine approach to cognitive decline. This is the best mentorship opportunity imaginable in the dementia reversal space. The role offers what most dementia care positions cannot: sustainable workload (8 patients daily vs 18-19 in managed care), meaningful outcomes where patients actually improve, and the infrastructure to comprehensively support both patients and caregivers. Key Responsibilities Provide dementia assessments and person-centered care plans for Medicare-eligible patients (65+) using Clinical Dementia Rating Scale and Functional Assessment Staging Tool Implement Bredesen Protocol (ReCODE) framework addressing root causes including nutritional deficiencies, infections, toxins, vascular health, and hormonal imbalances See average 8 patients daily (4 days/week) with 30-45 minute comprehensive follow-up visits every 6-12 weeks, building healing relationships where families express profound gratitude Lead CMS GUIDE Model implementation ensuring compliance with federal care delivery domains, quality metrics tracking, and performance-based adjustments Build formal partnerships with respite providers, meal delivery services, transportation organizations, home health agencies, and Area Agencies on Aging to connect patients with essential community resources Participate in weekly case review and training sessions (3 hours total) with Dr. Dale Bredesen and Pacific Neuroscience Institute collaborators Order and interpret comprehensive diagnostic testing (100+ biomarkers) with in-office lab capabilities and LVN support

Required Qualifications Active California Nurse Practitioner license (Family NP, Adult-Gerontology Primary Care NP, or Acute Care NP) Board certification with ability to bill Medicare from day one Genuine passion for geriatric population - must authentically enjoy working with elderly patients, not just tolerate them Enthusiasm for integrative/functional medicine approaches including nutrition, lifestyle interventions, and root-cause analysis beyond pharmaceuticals (training provided if coming from conventional background)

Preferred Qualifications 3+ years post-licensure experience (new graduates considered with in-house MD supervision available) ReCODE 2.0 Practitioner Training by Dr. Dale Bredesen (employer will pay $2,000 training cost for right candidate) Direct geriatric or dementia care experience in any setting Integrative/functional medicine background (IFM certification, worked at integrative clinics) Bilingual Spanish/English to serve San Diego's diverse population

Compensation & Benefits Competitive compensation plus quarterly performance bonuses.
