Trinity Code of Conduct Form must be signed and on file. Standard i9 completed by the agency. Trinity Integrity and Audit Services Form signed within days prior to start. Trinity Non Employee Upload Form must be completed and uploaded. Trinity Specialty Specific Competency Testing must be completed within a year. Proof of COVID-19 vaccine, booster, declination or exemption required. Proof of MMR vaccination or primary source IgG titer required. Hepatitis B vaccination series proof required; titers not accepted. Influenza vaccine proof or declination required for specified dates. 2 Step PPD OR IGRA (Quantiferon/Tspot) required. Proof of Varicella vaccination or IgG titer required. Physical/Health Statement from exam within last year required. Resume, Skills Checklist within a year, and 2 Professional References required.
