Job Title: Registered Nurse – Home Health
Location: Lillington, NC, 27546
Job Type: 3 months Contract
Shift: M-F until shift is complete
On call info: On call required if requested
Travel Area: Moore, Lee and Chatham Counties
Productivity Units: 30 units
Expected hours: 40 hours in a week
Float Requirement: Yes
Local Pay range: $40-$44/hour
Travel Gross Pay Range: $2000 - $2200/Week for 40 hours

Position Summary:
Primary functions of the Registered Nurse are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency and assume the responsibility for coordination of care.
Essential Duties and Responsibilities:
Assess and monitor home health or hospice patients, developing and updating individualized Plans of Care based on physical, psychosocial, and environmental needs.
Maintain accurate and timely documentation of patient care, ensuring quality, continuity, and compliance with regulatory and third-party payer requirements.
Educate patients and families on medications, care techniques, safety, and self-care based on age, condition, and readiness to learn.
Supervise and guide support staff (HHA/STNA/LVN) and coordinate referrals to other healthcare agencies as needed.
Perform on-call duties, participate in interdisciplinary team meetings, and communicate effectively with all parties involved in patient care.
Required Knowledge, Skills & Abilities:
Effectively assesses and responds to patient and family needs in diverse care settings.
Demonstrates strong communication, organizational, and critical nursing skills, including wound care, phlebotomy, and IV therapy.
Competent in using and maintaining medical equipment, catheters, and administering respiratory therapy.
Maintains current licensure, confidentiality, and meets continuing education requirements.
Works independently and collaboratively, adapting to flexible schedules and local travel.
Required Qualifications:
License/Certification:
Current state license as a Registered Nurse - Required
CHPN certification - Preferred
Current BLS certification - Required
Current driver's license - Required
Must have reliable transportation and insurance - Required
Experience:
Homecare Homebase Software Experience - Required
OASIS Experience - Required
1-2 years' experience as a Registered Nurse in a clinical care setting - Required
2+ years' Home health experience - Required.
Education:
Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program - Required
Bachelors of Science in Nursing - Preferred
