Position SummaryJoin a dedicated team delivering superior nursing, hospice, and rehabilitative care to over 70 towns across Vermont and New Hampshire. As an RN Case Manager, you aren't just a visitor; you are an integral part of the patient's care team, providing the clinical expertise and emotional support that allows patients to remain in their homes with dignity.?? Key Responsibilities??? Case Management: Screen, evaluate, and treat patients while developing comprehensive care plans in collaboration with physicians and families.?? Interdisciplinary Coordination: Act as the hub for the patient's care, communicating with therapists, social workers, and primary caregivers.?? Clinical Assessment: Provide ongoing evaluation of functional and emotional status, identifying progress or the need for plan revision.????? Education: Teach patients and families self-care techniques, medication management, and dietary instructions as ordered.?? Territory Coverage: Provide services in areas including Lebanon (NH), White River Junction (VT), Springfield (VT), and surrounding communities.?? Minimum Qualifications?? Education: Graduate of an accredited school of nursing.?? Experience: Minimum 2 years of dedicated experience in Home Health and Hospice.?? Licensure: Must hold active RN licenses in BOTH Vermont and New Hampshire or a valid unencumbered Compact License.?? Logistics: Valid driver's license, reliable transportation, and auto insurance (Travel between towns is required).??
