As a key member of our wound care team, you will: โ€ข Conduct thorough assessments and develop personalized treatment plans that address underlying health issues and co-morbidities โ€ข Deliver hands-on, bedside wound care to patients in skilled nursing and assisted living facilities โ€ข Lead the charge on wound assessment, treatment planning, and advanced procedures โ€ข Harness cutting-edge tech and evidence-based protocols to accelerate healing โ€ข Guide smart, effective decisions based on a holistic approach in collaboration with facility teams and families โ€ข Keep your charting sharp and timely in our customized EMR โ€ข Help drive quality initiatives and support our mission of raising the bar in post-acute care

Qualifications: โ€ข MD or DO with active and unrestricted IL license โ€ข Board Certification in Internal Medicine, Family Medicine, Surgery, or similar โ€ข 2 years of physician experience โ€ข At least 2 years of wound care experience โ€ข Previous experience in SNFs, long-term care, or rehabilitation facilities โ€ข IL prescriptive authority & DEA registrations โ€ข Ability to travel daily to assigned facilities in Chicago โ€ข Comfortable with EMR, mobile, and laptop documentation โ€ข Strong interpersonal skills โ€” collaborative, communicative, and adaptable
