As a key member of our wound care team, you will:
โข Conduct thorough assessments and develop personalized treatment plans that address underlying health
issues and co-morbidities
โข Deliver hands-on, bedside wound care to patients in skilled nursing and assisted living facilities
โข Lead the charge on wound assessment, treatment planning, and advanced procedures
โข Harness cutting-edge tech and evidence-based protocols to accelerate healing
โข Guide smart, effective decisions based on a holistic approach in collaboration with facility teams and
families
โข Keep your charting sharp and timely in our customized EMR
โข Help drive quality initiatives and support our mission of raising the bar in post-acute care
Qualifications:
โข MD or DO with active and unrestricted IL license
โข Board Certification in Internal Medicine, Family Medicine, Surgery, or similar
โข 2 years of physician experience
โข At least 2 years of wound care experience
โข Previous experience in SNFs, long-term care, or rehabilitation facilities
โข IL prescriptive authority & DEA registrations
โข Ability to travel daily to assigned facilities in Chicago
โข Comfortable with EMR, mobile, and laptop documentation
โข Strong interpersonal skills โ collaborative, communicative, and adaptable
