The Inpatient RN Care Manager provides professional, comprehensive, patient-centric care management services for at-risk patients in an acute care environment. The Care Manager RN is responsible for assessment and planning, care coordination, resource utilization management/review, discharge planning, documentation of interventions, regulatory compliance, and patient advocacy. This role functions in a unionized environment and works closely with care management assistants and a supportive leadership team.
Key Responsibilities
Assess, plan, and manage care for at-risk patients in acute care settings.
Coordinate multidisciplinary care and optimize resource utilization.
Conduct discharge planning and follow-up to ensure continuity of care.
Document patient interventions accurately and maintain regulatory compliance.
Serve as a patient advocate and provide education to patients and families.
Collaborate with physicians, nursing staff, and other healthcare professionals.
Promote a culture of patient-focused, whole-person care.
Participate in professional development and care management programs (e.g., TIPS or Case Management Orientation).
Required Qualifications
Associateโs Degree in Nursing (ADN) or Nursing diploma upon hire.
Washington Registered Nurse (RN) License.
Minimum 2 years of acute care experience in a Medical-Surgical or Inpatient setting.
Preferred Qualifications
Bachelorโs Degree in Nursing or higher (to be obtained within 3 years of hire).
National certification in care management or area of specialty.
1 year of care management or utilization review experience in any setting or completion of TIPS / Case Management Orientation Program.
