Promise Care Manager Location- Dover, DE

Job Summary Care Manager within the Promise Program, working as part of an interdisciplinary team to support individuals with severe and persistent mental illness, including those with co-occurring disorders. Responsible for conducting comprehensive assessments, developing individualized recovery plans, coordinating appropriate levels of care, and ensuring services are delivered in compliance with contract standards. Provides community-based support through beneficiary visits, documentation, and collaboration with a broad network of community providers. Serves as a liaison between clients, providers, and internal teams, represents the program in meetings, and assists with front-desk coverage and call screening as needed. Occasionally transports clients to appointments using a state vehicle.

Job Description: The incumbent will work as part of an interdisciplinary team in the Promise Program serving individuals with severe and persistent mental illness including those with Co-occuring disorders. The incumbent will act as a Care Manager in performing assessments, helping to develop individualized recovery plans, determining appropriate levels of care, placing individuals in those levels of care and ensuring that care is delivered according to contract standards. Duties will include visiting beneficiaries in the community, charting, acting as a liaison with a large numbers of community providers. You may be asked to cover meetings and represent our office and at times assist in covering the front desk to screen calls. There may be a few occasions you will assist in transporting clients in a state vehicle to appointments.

Qualifications: Bachelor degree and valid driver's license, experience in the mental health field is beneficial.
