Assessor (Dayton)

RN, LPN, or LSW to conduct multifaceted comprehensive assessments of consumers to identify unmet needs, link to long-term care options and determine eligibility for a variety of service alternatives, including but not limited to PASSPORT Home Care enrollment.

RN, LPN, or LSW with a minimum of one year prior experience in home care for elderly and/or disabled adults, home health care, medical social work, geriatrics/gerontology or other substitute as approved by the ODA.
