03/31/2026 Job Description: The RN Case Manager is responsible for managing patient care with a focus on providing coordinated care as a Care Coordinator in a Dignity Health Hospital setting. They will handle a patient population including cardiac, surgical, oncology, and critical care with daily caseloads ranging from 15-25 patients. Responsibilities: Manage and coordinate patient care for a diverse patient population. Float to all hospital units excluding surgery to provide care as required. Requirements: Recent California Dignity Health Hospital experience as a Care Coordinator. BLS Certification required. Must not have been directly employed by CommonSpirit, CHI, or Dignity in the past year unless through outside firms.
