Find your calling at Mercy! The RN II Advanced Primary Care role provides virtual care to patients that benefit from ongoing chronic disease management. In partnership with a multidisciplinary team, including physicians and advance practitioners, the RN is responsible for creating patient relationships, monitoring vital sign trends, conducting patient visits in between regular office visits, assisting in panel management of patients and providing individualized care to improve patient outcomes.
