What You???ll Do Provide primary care to 15???20 patients/day in post-acute care settings.

Collaborate with an interdisciplinary team to improve patient outcomes.

Coordinate with hospice, palliative care, and home care agencies to ensure smooth care transitions.

Keep accurate, timely records in Gehrimed EMR.
