Care Manager (RN – Pediatrics)
Florida - Remote + Field Visits)
Florida
Monday–Friday, 8 AM–5 PM
We are looking for an experienced Pediatric Registered Nurse to join our Extended Care Coordination (ECC) team. This role focuses on supporting medically complex children and partnering closely with their families to help them receive the right care at the right time. If you have strong pediatric clinical experience, excellent communication skills, and a passion for helping families navigate care, this may be a great fit.
What You’ll Do
Conduct monthly outreach to parents/caregivers, including:
Monthly update calls
Monthly face-to-face visits (or virtual visits if declined)
Annual assessments
Gather and document detailed clinical and social updates to complete required
ECC notes
each month.
Coordinate with
DME vendors, providers, and PDN agencies
to resolve equipment or care issues reported by families.
Participate in
care plan review calls every six months
with PDN agencies.
Maintain clear, timely, and accurate documentation to meet CMS and Sunshine Health standards.
Build supportive relationships with parents, troubleshoot concerns, and help guide families through complex care needs.
What Success Looks Like
Completing all required monthly contacts, visits, assessments, and ECC documentation on time.
Communicating professionally with families, even in challenging situations.
Collaborating effectively with providers, PDN agencies, and internal Sunshine Health teams.
Maintaining organized, thorough, and audit-ready documentation.

Comprehensive
4-week training program
before receiving a caseload.
Strong, collaborative team culture—tight-knit, supportive, and always willing to help.
Ongoing coaching from experienced supervisors and clinical leadership.
What We’re Looking For
Active RN license (Florida)
Minimum 2 years of continuous pediatric RN experience
Pediatric ICU, PICU, or pediatric home health strongly preferred
Comfortable completing
field visits
within Region 7
Proficient with Microsoft Word and Excel
Strong documentation and communication skills
Preferred
BSN (ASN also accepted)
Prior case management or care coordination experience
