Roles & Responsibilities Coordinate comprehensive case management for adults and geriatric patients to ensure safe, timely transitions Perform utilization review and needs assessments to align care with best practices and payer requirements Develop and implement individualized plans of care and discharge plans to improve patient outcomes Order durable medical equipment and arrange community resources to support post-discharge needs Collaborate with interdisciplinary teams including social services, transportation, and interpreters Communicate clearly and diplomatically with patients, families, and clinical teams to resolve barriers Document care and utilization findings accurately and professionally to meet regulatory standards Problem solve creatively and independently in a high-volume environment to keep care moving

Qualifications Current CA RN license required AHA BLS certification required Minimum 2 years of recent acute hospital experience required Masters degree preferred COVID-19 vaccine plus booster required; medical or religious exemptions only Ability to float to other UCSF facilities as needed Strong written and oral communication skills, organization, diplomacy, and analytical ability

Education Masters Degree (preferred)

Knowledge & Skills Expertise in case management, care coordination, and discharge planning Experience with utilization review and needs assessment processes Ability to create and manage plans of care and order DME Knowledge of workers compensation case management Familiarity with regulatory standards such as CMS, HIPAA, and The Joint Commission Comfort working in a fast-paced acute hospital environment and collaborating with interdisciplinary teams
