Responsibilities:

Conduct and complete resident assessments (MDS) accurately and on time Collaborate with the care team to develop individualized care plans Monitor and document resident progress Participate in Quality Assurance and Performance Improvement (QAPI) Communicate with residents, families, and healthcare providers Stay current on MDS regulations and guidelines

Compensation & Benefits:

Wage Range: $37.00 - $41.00/hr.

Requirements:

RN license and RAC-CT certification in good standings with no restrictions Knowledge of MDS 3.0 and care planning Strong organizational and communication skills Team player with a commitment to high-quality resident care
