Dietitian

Job Responsibilities: Develop and implement Client nutrition services program in collaboration with the Interdisciplinary Team (IDT). Conduct nutrition assessments to determine participant needs, preferences, and goals. Participate in IDT meetings and contribute to participant care plans. Deliver and document nutrition interventions per physician orders and care plans. Implement, monitor, and ensure compliance with special diets and meal intake. Provide nutrition counseling to participants and families. Oversee menu planning, meal delivery, and ensure regulatory and safety compliance. Coordinate and oversee nutritional supplements as ordered. Train day center staff on meal delivery and nutrition services as needed. Support care transitions across settings (hospital, SNF, home, etc.). Assist with intake and enrollment processes for new participants. Communicate participant condition changes to IDT promptly. Complete documentation accurately and on time. Maintain confidentiality and follow Client policies and procedures. Participate in quality improvement initiatives, staff training, and meetings. Maintain flexibility in scheduling, including potential evenings, nights, or weekends. Complete all required trainings, education, and occupational health screenings. Other duties as assigned. Requirements: Minimum 4 years’ experience managing nutrition services in a similar setting. Minimum 1 year experience working with frail or elderly populations. Registered Dietitian (RD) credential through Commission on Dietetic Registration (CDR). BLS certification. Legally authorized to work at the job location. Ability to work varied schedules if required. Reliable transportation required (no use of personal vehicle for participants). Preferences: Bachelor’s Degree preferred. Strong leadership, mentoring, organizational, and communication skills. Ability to work independently with minimal supervision. Experience in fast-paced environments and diverse populations. Commitment to serving vulnerable senior populations.
