We’re seeking an experienced
Nurse Manager, Care Management Program
to join the Population Health team. This leader will guide a comprehensive care management program that supports pediatric, adult, and maternal patients—many engaged in value‑based and Medicaid programs across the Commonwealth.
About the Role
As Nurse Manager, you will oversee daily operations for a multidisciplinary care management team supporting patients with complex medical, behavioral, and social needs. You’ll lead RN Care Managers, Behavioral Health Care Managers, and Community Health Workers while championing program quality, compliance, and excellence.
This role is ideal for a collaborative nurse leader with strong knowledge of Massachusetts care‑management regulations and a passion for serving underserved communities.
Key Responsibilities
Provide day‑to‑day operational leadership and ensure compliance with state, federal, and contractual requirements
Improve workflows in response to new contracts, policies, and program evaluations
Monitor dashboards, performance metrics, and caseload distribution
Use data insights to drive efficiency and improve patient outcomes
Partner with Population Health leadership on quality and staff development initiatives
Conduct chart audits, offer supervision, and complete performance evaluations
Communicate workflow and policy updates to frontline staff
Deliver training and coaching to reinforce best practices
Maintain proficiency in care‑management technology platforms
Lead clinical rounds and participate in interdisciplinary discussions
Oversee staffing, including hiring, onboarding, scheduling, and coverage planning
Qualifications
Bachelor’s degree in Nursing (BSN) and active Massachusetts RN license
Case Management certification preferred (CCM, ACM, or similar)
3–5 years of leadership experience in care management or population health, ideally in value‑based or managed‑care settings
Experience supporting Medicaid populations and patients with complex medical, behavioral, and social needs
Proven ability to lead interdisciplinary teams and collaborate with providers and community partners
