Job Details
Assesses patients on admission within specified time frames considering physical, psychosocial, cultural, and spiritual needs.
Reassesses each patient's health needs whenever warranted and at least every shift using nursing processes.
Establishes and updates plan of care based on data from all appropriate sources.
Administers oral, topical, and parenteral medications using the 7 rights.
Assesses and documents effectiveness and untoward effects of medications and treatments.
Performs treatments and procedures following nursing practice, hospital policy, sterile technique, standard precautions, and patient privacy.
Documents response to care, pain management, discharge planning, education, and progress towards goals.
Educates patient/family based on assessed needs using 'Krames' where appropriate.
Interprets and analyzes diagnostic data and intervenes appropriately.
Communicates changes in patient's clinical condition promptly to physicians and healthcare team using SBAR communication for handoffs.
Demonstrates consideration of patient rights including confidentiality, safety, and participation in plan of care.
Evaluates and documents patient care and effectiveness of patient teaching.
Participates in multidisciplinary care planning.
Demonstrates knowledge and management of patient restraints per policy.
Follows National Patient Safety Goals and hospital policies including use of two patient identifiers.
Understands Disaster Plan and own role, including activation of Rapid Response Team.
Completes orientation to Life Safety Standards including Code Blue, Code Pink, Haz Mat Spills, Code Red, and Oxygen shut off valve.
Follows hospital infection control policies.
Demonstrates knowledge of blood transfusion policy and documentation.
Understands Chain of Command and accesses it appropriately.
Follows guidelines for Core Measures and vaccine control.
Documents appropriately regarding initial assessment, multi-disciplinary care plan, progress notes, ICU flow sheet, medication record, medication reconciliation, critical values, and telephone orders.
Uses Pyxis appropriately for narcotics and follows policy for administration of high-risk medications.
Able to use pressure monitoring, bedside cardiac monitors, recognize cardiac dysrhythmias, and specialty beds.
Manages pain and sedation protocol.
Follows early mobility protocol and protocols for sepsis, ventilator-associated pneumonia, pressure ulcers, and central lines.
Demonstrates knowledge of nurse-collected specimens for type and cross/screen.
Accurately assesses, plans, implements, and evaluates care of critically ill patients.
Accesses appropriate resources when assistance is needed.
Locates and accesses unit-specific policies as appropriate.
Traveler will float to ICU, ED, PACU to care for ICU patients and also float to care for full IMC patients in the ED, 2500 and 3100 units.
Job Requirements
California RN License required.
5 years of recent ICU RN experience in acute care facility preferred, including SICU, MICU, and Trauma.
Knowledge and ability to manage patients in restraints per policy.
Ability to use pressure monitoring, bedside cardiac monitors, and recognize cardiac dysrhythmias.
Familiarity with pain and sedation protocols, early mobility protocol, and protocols for sepsis, ventilator-associated pneumonia, pressure ulcers, and central lines.
Knowledge of nurse-collected specimens for type and cross/screen.
Ability to document accurately and legibly including date, time, signature, and title.
