Nurse Practitioner (NP) or Physician Assistant (PA)
Post-Acute Care Tucson, Arizona Full-Time
We are seeking a compassionate and driven Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing post-acute care team in the Tucson, Arizona area. This is an excellent opportunity for an experienced clinician who values autonomy, collaboration, and meaningful patient impact.
This full-time position (5 days per week) involves rounding in skilled nursing facilities, supported by a strong clinical and operational leadership team. We are specifically seeking a provider with at least two years of NP or PA experience, ideally with a background in Physical Medicine & Rehabilitation (PM&R) or physiatry. Why Join Us?

Estimated annual earnings: $143,000 - $171,000

Key Responsibilities
Perform comprehensive patient assessments, including medical histories and physical examinations
Develop, implement, and adjust individualized care plans
Monitor patient progress and coordinate care to optimize outcomes
Collaborate closely with physicians, nursing staff, and interdisciplinary healthcare teams
Educate patients and families on health management and preventive strategies
Maintain accurate and timely medical documentation in compliance with regulatory standards
Qualifications
Active Arizona NP or PA license
Current DEA license
Minimum 2 years of NP or PA experience preferred
Experience in post-acute, acute care, emergency department, rehabilitation, or clinic settings preferred
Strong clinical foundation and knowledge of healthcare regulatory standards
Excellent communication and interpersonal skills
Collaborative, team-oriented approach with a positive, patient-centered mindset
