Each patient receives a dynamic wellness plan that is actively managed monthly—without fixed check-ins—ensuring the right care at the right time. Medicare covers most program costs, enabling patients to live better with consistent clinical oversight.
Position Summary
The Supervising Physician provides clinical oversight and consultative direction across Wellness Care’s national CCM programs. This role is consult-based and focused on regulatory compliance, care plan validation, clinical escalation, and quality assurance rather than primary longitudinal care.
The physician ensures that all care plans meet Medicare requirements, clinical best practices, and board-level standards while supporting RN Care Managers and Care Professionals in delivering structured, defensible, and audit-ready care.
This role does not function as a traditional primary care provider. It serves as the clinical anchor and oversight authority for the four-level care delivery model.
Core Responsibilities
Clinical Oversight & Governance
Provide supervision and clinical oversight for CCM services.
Review and approve care plans required by Medicare regulations.
Ensure care plans align with evidence-based medicine and patient-specific risk profiles.
Serve as an escalation point for complex cases requiring physician-level review.
Maintain documentation standards consistent with CMS requirements.
Medicare Compliance & Regulatory Alignment
Ensure compliance with CCM (99490, 99487, 99491, etc.) billing requirements.
Validate medical necessity for enrolled patients.
Support audit readiness and documentation defensibility.
Partner with compliance and coding teams to address regulatory risk.
Assist in policy development for evolving CMS rules.
Care Plan Integrity & Quality Assurance
Provide clinical input on wellness-based care plan templates.
Guide chronic condition management protocols.
Support development of condition-specific pathways.
Review trends in outcomes and provide intervention where needed.
Participate in internal quality reviews.
Team Collaboration
Partner with RN Care Managers and Care Professionals to ensure clarity of scope.
Provide structured consult guidance for escalated patient cases.
Support specialist integration into care plans.
Contribute to training and clinical education initiatives.
Program Development
Advise leadership on clinical model evolution.
Contribute to scaling strategy for multi-state expansion.
Support integration of telehealth and remote monitoring initiatives.
Scope of Engagement
This is a consult-based role. Responsibilities include:
Scheduled clinical review sessions
Escalation case consultations
Policy and documentation review
Availability for clinical questions within defined response timelines
Participation in quality or compliance meetings as needed
This role does not require daily direct patient visits unless clinically necessary within the TCM framework.
Qualifications
Required:
MD or DO
Active, unrestricted license in all 50 states
Medicare enrollment and good standing
Experience with CCM or value-based care models
Understanding of CMS documentation and billing requirements
Experience working in telehealth or remote care environments
Preferred:
Medicare affiliation
Experience supervising multi-level care teams
Experience in geriatrics, internal medicine, or family medicine
Contract rate starts at $1500 per day DOE.
Compensation structured as consult-based hourly pay aligned with national supervisory physician benchmarks, reflecting the scope of Medicare oversight and multi-state licensure requirements.
