Nurse Practitioner / Internal Medicine / California / Locum Tenens / Locums NP/PA Adult Medicine Job

We are seeking a certified Nurse Practitioner

This full-time assignment offers a great opportunity for an NP who thrives in a collaborative, team-based environment and enjoys a mix of clinic and home-visit responsibilities.

Position Details: Start Date: December 9, 2024 End Date: March 31, 2025, with potential for extension.

Schedule: 3 days per week in the clinic.

Remaining days include documentation, home visits, and skilled nursing facility visits.

90% clinic-based; 10% fieldwork.

No inpatient or hospital privileges required.

Call: Not required but optional.

After-hours nurse or operations director triages first before contacting the provider.

Key Responsibilities: Provide patient care independently in the clinic and during home or skilled nursing facility visits.

Handle an average of 4 home visits per week (majority conducted by an RN).

Deliver high-quality care to a geriatric patient population in a team-based care model.

Requirements: Licensure: Active California NP license required.

Certification: Basic Life Support (BLS) certification.

Experience: Preferred experience in long-term care or geriatric medicine.

Ability to work autonomously in a clinical setting.

EMR: Familiarity with Epic preferred; onboarding for access provided.
