Requirements:

Active RN license Graduate of accredited nursing program (BSN preferred) BLS Previous case management or acute care experience preferred Knowledge of discharge planning and insurance guidelines Strong communication, organization, and problem-solving skills

Responsibilities:

Assess patient needs and develop individualized care plans Coordinate care across providers, services, and settings Facilitate discharge planning and transitions of care Collaborate with interdisciplinary teams and community resources Monitor patient progress and adjust plans as needed Ensure appropriate utilization of services and resources Educate patients and families on treatment plans and follow-up care Document care coordination activities in the EMR
