Company Description: Connections Care Home Consultants is a CalAIM Community Support Provider dedicated to assisting Private-Paid Clients and Medi-Cal Recipients with transitions between Skilled Nursing Facilities (SNFs) and Assisted Living environments. Partnering with various Managed Care Plans, the organization supports SNF Diversion and Transition programs as part of the CalAIM initiative. Connections Care Home Consultants proudly serves several counties across California, including Los Angeles, Orange, Sacramento, San Diego, Ventura, San Bernardino, Riverside, and Antelope.

Position Summary: We are looking for a friendly, outgoing, and organized Bilingual (Spanish/English) Health Services Intake & Customer Service Specialist to serve as the "voice" of our agency. Working closely with our Community Support Manager, you will be the first point of contact for members and families inquiring about the CalAIM program and assisted living that call inquiring onboarding as a new provider or regarding claim status. This is not a call-center role; it is a relationship-building role. You will guide families through the entire intake process, explaining the benefits of CalAIM, helping them complete applications, and managing their journey from the first phone call to enrollment.

What You Will Do (Key Responsibilities): First Point of Contact:

Field incoming calls from members and families interested in the CalAIM program in and most be completely proficient in English and Spanish. You will answer questions, ease concerns, and explain how the program can cover assisted living costs. Intake Management:

Manage the entire intake lifecycle. This includes screening potential clients for eligibility and gathering necessary demographic and medical information and confirming Application Assistance:

Actively assist members and their families in filling out program applications, ensuring all data is accurate and complete to prevent delays. Program Education:

Become a subject matter expert on our specific CalAIM Community Supports, clearly explaining the scope of services to families who may be confused by the healthcare system. Follow-Up:

Maintain a pipeline of interested members, performing consistent follow-up calls and emails to ensure applications are returned and processed. Administrative Support: Collaborate with the CS Manager to ensure all client files are updated in real-time.

What You Bring (Qualifications & Skills): Bilingual Fluency:

Must be fluent in both English and Spanish (written and verbal). Education:

An Associateโ€™s (AA) or Bachelorโ€™s degree is required. Experience:

Minimum of 2 years of experience in health services customer service, patient intake, or a similar healthcare role. Tech Proficiency: -Must be proficient in Microsoft Word. -Must be familiar with Google Drive and Google Docs. -Must possess strong general computer skills. Remote Work Setup:

You must have your own reliable computer and a high- speed internet connection.

Important Position Details: Wage:

$40,000 salary Work Schedule:

Our business is based in California. This is a remote position, but you must be available to work Pacific Standard Time (PST) business hours. This translates to 12:00 PM โ€“ 8:00 PM Eastern Standard Time (EST), Monday through Friday. Please ensure you can accommodate this schedule before applying.
