nonclinical APP to make 3-5 days post discharge calls to complex pts, ensure med rec scripts are filled and picked up/follow up appointment is made with PCP /specialists Position Details: Hartford Hospital is seeking a Physician Assistant or Nurse Practitioner to join their Department of Medicine team! We are looking for a non-clinical APP to conduct outreach post discharge calls to

complex patients, ensuring discharge instructions are adhered to. Go over medication reconciliation lists and confirm prescriptions are filled and picked up. Establish follow up with PCP/specialists.

Nurse Practitioner and Physician Assistants Role Scope: Background •

High readmission rates negatively impact patient outcomes and healthcare costs. •

CMS considers readmission rates for COPD, AMI, PNA, and CVA when evaluating hospitals.

Objective •

Utilize Epic AI model to identify high-risk patients at discharge •

Reduce readmission rates for high risk, high VVI score patients •

Target patients with ≥22% predicted risk of readmission •

Focus on those discharged home, not to a facility

Qualifications

Requirements: Licensure: Unrestricted State of Connecticut Medical License (or eligible) Training: Certified or board-eligible Physician Assistant or Acute Care Nurse Practitioner Experience: 5 years’ experience
