Required:

1+ Year Hospital Experience working with adults with mental illness

Education:

Master's degree in social work

License & Certification:

Valid Licensed Master of Social Work (LMSW) license issued by NYSED

Duties: Conduct psychosocial assessments to identify patients’ social, financial, and environmental needs Assist clients with care coordination, discharge planning, and community resource referrals Provide case management support, including follow-ups and service linkage Advocate for patients regarding housing, insurance, benefits, transportation, and social services
