Agency Social Worker - Reablement Service - Brent Council (up to 12 month assignment)

Our client, Brent Council, is seeking an agency Social Worker for its Reablement service. This role will last 6 months initially but more likely up to 12 months and is paid at £32 per hour. The roles will involve the following: An integrated service that delivers fast-paced responses to hospital discharges, alongside reablement and social care support through a short-term intervention model. Our team ethos centres on promoting and enabling independence, and we maintain strong working relationships with partners and colleagues across hubs and intermediate care services. Completing Care Act assessments Care and Support plans Multi-agency working Working in a fast-paced way, taking up to 3 allocations per week and actioning them quickly Visiting people out in the community – no telephone-based assessments Working closely with Reablement providers and getting feedback from them Working in a team which includes Occupational Therapists and OT assistants Liaising with GPs, physiotherapists etc. Using Mosaic and benefitting from the Magic Notes to lessen admin tasks Please let me know if you are interested? Requirements of the role: Qualified Social Worker Fully Social Work England registered Must have post-qualified experience working within a local authority adult social care setting Does not require specialist Reablement experience, but the ideal candidate will have worked with adults with physical disabilities and will have demonstrable experience in a fast-paced setting such as hospital discharge, front door team, duty etc. Comfortable being in the local area 3 days per week for visits and some office presence Please apply online or contact Lindsey Morgan ASAP to discuss in more detail.
