- Bachelor’s degree in a human services field
- Associate’s degree in a human services field and two years of full‑time direct service experience
- High school diploma/GED and four years of full‑time human services experience
Experience: Experience providing direct services to individuals with mental health needs, including counseling, case management, crisis intervention, and service coordination.
Certifications/Licenses:
- No specific license required.
- Must be able to travel within the community; a valid driver’s license may be preferred depending on assignment.
Work Eligibility: Must be authorized to work in the U.S.
Work Location Requirement: Onsite / Community‑based field work (regular travel to consumer residences).

About the Role: This role functions as part of an interdisciplinary team supporting individuals in maintaining stable, independent living in the community. The position involves home visits, counseling, service coordination, crisis intervention, and advocacy to help consumers access medical, social, educational, vocational, and financial resources. Most duties are performed independently with general supervision.
Primary Responsibilities
Conduct home visits to provide supportive counseling and independent living assistance.
Help consumers modify behaviors, build insight, and develop coping skills.
Assess needs, gather medical/psychosocial information, and develop individualized service plans.
Maintain accurate case records, documentation, assessments, and quarterly reviews.
Provide 1:1 and group counseling using cognitive‑behavioral and skills‑training techniques.
Coordinate services with care managers, ICL staff, and external providers.
Assist consumers in accessing community resources, benefits, and vocational opportunities.
Perform crisis assessment and intervention; refer to appropriate services.
Accompany consumers to medical, social service, and government appointments.
Report incidents and complete incident documentation per policy.
Conduct safety and cleanliness inspections of consumer residences.
Plan and facilitate recreational or social activities.
Participate in meetings, supervision, and required training.
Advocate for consumer needs within ICL and with external providers.
Perform other related duties as assigned.
