About the Role
The Care Coordinator is responsible for ensuring the smooth delivery of care services by managing staff schedules and monitoring care calls. The role involves effective communication with clients and healthcare professionals, addressing concerns, and overseeing staff performance through analysis and monitoring.
Responsibilities
Creating staff rotas and managing schedules
Monitoring and covering care calls
Communicating with clients and healthcare professionals
Addressing staff and client concerns
Carrying out staff performance analysis and mentoring
Managing care plans and reviews
Gathering feedback and implementing service improvements
Completing any additional tasks assigned by management
Qualifications
Any Graduate/Diploma
Previous work experience in customer service/support or as a coordinator. (Highly Preferred)
Required Skills
Communication Skills – strong verbal and written communication to liaise effectively with clients, families, staff, and healthcare professionals.
Problem-Solving & Decision Making – proactive in addressing staff and client concerns and resolving issues quickly.
Leadership & Mentoring – capability to monitor staff performance, provide feedback, and support professional development.
Attention to Detail – accurate record-keeping, reporting, and performance monitoring.
Client Service Orientation – empathetic and professional approach to client care and satisfaction.
Adaptability & Flexibility – able to respond to changing schedules, urgent cover needs, and management directives.
Teamwork & Collaboration – works effectively with colleagues across the care team and wider healthcare network.
IT & Administrative Skills – comfortable using scheduling systems, digital care platforms, and maintaining documentation.

Experience working in the Healthcare industry
