Social Work Assistant

Responsibilities You will provide support to the Medical Social Workers in obtaining patients’ social, familial and financial information to facilitate appropriate service referrals.

You will conduct home visits and interviews to ensure that patients receive continuity of care upon their discharge as well as to determine the psychosocial adjustment of patients in the community.

Requirements

GCE ‘O’/‘A’ level with at least 2 years’ relevant experience in social services; or Diploma preferably in Community Services or Human Services

Good team player with passion in working with people

Proficient in Microsoft Office applications
