What You’ll Do:

Work closely with Occupational Therapists to carry out care plans that help clients adapt, recover, and maintain daily living

Use interventions such as adaptive equipment education, fine motor rehab, and cognitive training

Encourage functional improvements through consistent client interaction and exercise guidance

Join interdisciplinary care planning to ensure coordinated, high-quality services

Ensure session documentation meets quality, compliance, and goal-setting standards

Qualifications:

Current Minnesota

COTA license

Reliable vehicle and valid MN driver’s license

Strong documentation and team communication skills

Passion for

personalized, meaningful care in the home
