Reporting to the Geographic Medical Leader and the Regional Medical Director, Long-Term Care & Assisted Living, the Long-Term Care Community Medical Director is a physician who has a passion for caring for the elderly and has the following relationships:
member of the local Division of Family Practice Long-Term Care Initiative (LTCI)
collaborates with the local LTCI
collaborates with the Regional Medical Director, Long-Term Care & Assisted Living (LTCAL) and members of the Long-Term Care and Assisted Living Physician Leadership Team
collaborates with the local long-term care community leadership
Duties and Responsibilities
Clinical Quality of Patient Care: Regular review of quarterly clinical performance indicators provided by Fraser Health Long-Term Care and Assisted Living to support clinical teaching and clinical research activities at the care community.
Participation in the evaluation of the efficiency, quality and delivery of services that require the professional expertise of a physician (e.g. patient quality reviews participation in medical audits; peer and interdisciplinary reviews; chart reviews; and incident report reviews).
Provide medical leadership to physicians at the care community by liaising between the physicians, Fraser Health Long-Term Care Physician Services, and care community leadership on matters pertaining to clinical quality and direct and indirect patient care.
Clinical Best Practice Expectations: Support physicians in meeting the Division of Family Practice Long-Term Care Initiative’s five clinical best practice expectations: 24/7 availability and on-site attendance; proactive visits to residents; meaningful medication reviews; complete documentation; attendance at case conferences.
Clinically Related Teaching and Research: Attend the site frequently and work closely with clinical staff and leadership to provide education on and help implement clinically related Fraser Health Long-Term Care strategic priorities. As possible, support and participate in opportunities for on-site physician education/mentoring of clinical staff, physicians interested in long-term care, and family practice residents.
