Key Responsibilities
Provides orientation to new patients to the program’s rules, policies, procedures, and patients' rights.
Conducts intakes, discharge planning, initial assessments, and initial treatment plans professionally and within time guidelines as required by applicable federal and state regulation, CARF guidelines and Pinnacle policies.
Involves patients in assessment, treatment planning, and intervention to give them a sense of entitlement to their own recovery.
Administers the biopsychosocial assessment, evaluating all areas of patients’ lives at intake and on an ongoing basis.
Recognizes and utilizes patients’ Strengths, Needs, Ability, and Preferences (SNAP) to develop individualized treatment plans.
Determines the clinical necessity of counseling services based on assessment and evaluation.
Monitors and documents changes in patient circumstances, identifies relapse indicators, and promotes relapse prevention interventions in an early stage.
Complies with Medical Quality Assurance and Clinical Risk Management Policies, including:
Evaluate high-risk conditions and use screening tools (e.g., BAM-R, C-SSRS, GAD-7, PHQ-9) to determine severity.
Assess risk factors and consider appropriate treatment settings; refer as needed and obtain release waivers for information sharing when appropriate.
Flag high-risk patients to Medical Directors and Regional Clinical Leadership and coordinate referrals as needed.
Review physician notes to ensure high-risk factors are noted and addressed, and communicate with supervising staff as required.
Inform and circulate high-risk patient information among staff following treatment team decisions.
Provide professional counseling and referral services by maintaining referrals to specialized services and to community resources, and acting as liaison as necessary.
Deliver individual and group counseling as clinically needed, addressing issues identified in assessments and treatment plans.
Utilize advanced counseling techniques such as Cognitive Behavioral Therapy and Motivational Interviewing.
Schedule regular counseling sessions, issue appointment cards, and record sessions in the EHR scheduler.
Document counseling entries with signature and date per federal/state regulations, CARF guidelines, and Pinnacle policies.
Engage in Community Relations and Outreach.
Attend required trainings and case conferences with program physicians and physician assistants.
Collect research data when requested and observe urine sample collection when required.
Attend team meetings and complete training courses timely.
Other duties as assigned.
Qualifications
Counselor I: $20/hour to $21/hour
Possess an associate degree in a relevant field or an approved counselor certification, license, or certification; Kern County requires a bachelor’s degree or 75% of required counselor certification courses.
Possess counselor registration/certification by a recognized certifying organization; proof of registration required prior to providing counseling services.
0-2 years of experience.
Counselor II: $21/hour to $24/hour
Possess an associate degree or majority of required certification courses; Kern County requires a bachelor’s degree or 75% of required counselor certification courses.
Possess counselor registration/certification by a recognized certifying organization; proof of registration required prior to providing counseling services.
2-5 years of experience OR Master level.
Counselor III: $24/hour to $27/hour (Certified) & $27/hour - $30/hour (Licensed)
Degrees must include licensure, licensure-eligible, or certification. Clinical skills must be effective in complex cases.
Possess certification as an Alcohol and Other Drug Counselor with a CA approved certification agency or current credentials as licensed or license-eligible with the Board of Behavioral Sciences or the Board of Psychology. If license-eligible, must maintain the Board’s requirements and receive the necessary supervision.
Proof of licensure or certification by a state regulatory agency is required immediately; credentialing must occur prior to providing counseling services per the DHCS.
