The Role
We are currently seeking a compassionate, professional, and proactive Care Partner to join our dedicated team.
This role is pivotal in supporting Australians under the Support at Home Program through collaborative care planning, service coordination, and wellness‑focused support. Working closely with clients and their support networks, you will help co‑design care plans aligned with their goals, manage budgets, coordinate service delivery, and ensure high standards of quality, compliance, and client satisfaction.
Responsibilities and Tasks:
Develop, implement, and regularly review person‑centred care plans and budgets based on client assessments, goals, risks, and support needs, ensuring all services are culturally safe, trauma‑aware, and aligned with funding guidelines.
Work closely with clients, families, carers, and multidisciplinary teams to co‑design care plans, monitor progress, and adjust services in response to changing needs, ensuring continuity and quality of care.
Conduct comprehensive risk assessments, maintain compliance with Support at Home standards, and coordinate care transitions and case conferences to ensure safety, well‑being, and optimal client outcomes.
Assist clients in understanding and managing their individual budgets and co‑contributions, ensuring services remain within financial limits and aligned with their approved care plans.
Monitor, track, and approve expenditures, process claims and invoicing accurately, manage consumables, and ensure full compliance with financial policies and Support at Home operational procedures.
Maintain clear, detailed, and timely documentation of all client interactions, care plans, reviews, and service‑related meetings.
Record and manage participant notes, incidents, and complaints to ensure accountability and service quality.
Regularly review care notes to identify changes in participant needs or risks and take prompt, appropriate action.
Collaborate regularly with rostering, clinical, and care teams to ensure client well‑being and continuity of care.
Communicate openly and professionally with clients, families, and colleagues, and participate in team meetings and knowledge sharing.
Ensure full compliance with strengthened aged care quality standards, documentation, and reporting requirements.
Participate in WHS risk assessments, audits, and ongoing quality improvement initiatives.
Follow all WHS legislation, policies, and safety protocols to maintain a safe work environment.
Conduct regular safety and risk assessments of participants and their homes and participate in safety training.
Perform additional tasks as directed in line with organisational needs and priorities.
Uphold St Louis' values, culture, and philosophy through professional conduct and positive workplace contribution.
Provide wellness‑focused, re‑ablement‑based support and promote health education through information sharing and community resources.
Empower participants and their carers to make informed decisions, access aged care systems, and navigate support plan reviews and additional services.
Advocate for participants' rights by ensuring their voices are heard, addressing feedback and complaints, and continuously improving service quality and access.
Provide 24‑hour phone support on roster and coordinate emergency responses promptly.

Essential Qualifications, Work Experience & Clearances
Must hold a minimum Diploma or higher qualification in a relevant field.
At least 1 year of relevant work experience in aged care, home care, or a related community service setting.
Participation in formal and informal in‑service training programmes.
Current National Police Clearance.
Valid and unrestricted driver’s licence.
Overseas criminal convictions statutory declaration (form will be supplied during onboarding).
NDIS worker check.
Knowledge of Aged Care Quality Standards.
Employees will be subjected to random drug testing.
Skills and Knowledge:
Excellent communication and interpersonal skills, including with CALD communities.
Ability to work independently and collaboratively across multidisciplinary teams.
Solid understanding of the Support at Home Program and Strengthened Aged Care Quality Standards.
Time management, planning, and problem‑solving skills.
Report writing and basic financial management skills.
Knowledge of aged care services, casework procedures, and client advocacy.
Bicultural or bilingual ability is highly regarded.
Deliver care in line with the Strengthened Aged Care Quality Standards.
Comply with all applicable Work Health & Safety (WHS) legislation and policies.
Maintain accurate documentation, escalare concerns appropriately, and actively participate in internal audits.
Engage in ongoing professional development and contribute to service improvement initiatives.
Promote a safe, inclusive, and respectful care environment.
Remuneration:
AUD $37.35 - $46.69 per hour.
Hours
Monday to Friday, 9:00 AM - 5:00 PM (38 hours/week).
Special Conditions
The successful candidate will be required to participate in a 24‑hour on‑call support roster, providing after‑hours assistance as scheduled.
Must be available and responsive to emergency coordination requirements when rostered.
Will be expected to uphold the values, philosophy, and professional conduct standards of St Louis Aged Care in all duties.
