Through the Support at Home program, we deliver personalised care that meets each participantsneeds, goals, and circumstances.

About the Role We are seeking an experienced Allied Health qualified and APHRA registered individual to join our Support at Home team.

This role is central to ensuring participantsreceive the right services, aids, equipment and home modifications to support, improve and maintain independence and quality of life.

You will be engaging with participants, families and stakeholders and conduct in-depth assessments and coordinate personalised care plans. Provide ongoing advice, guidance and care navigation. Ensure Quality care, risk management and continuous improvement.

About You

Allied Health qualified and AHPRA registered

Experience in Restorative Care, Allied Health or Community Services

Knowledge of Aged Care Quality Standards and the Support at Home program

Attention to detail in clinical or case care, and all documentation, budgeting and regulations the sector requires

Ability to work independently and collaboratively in a multi-disciplinary team

Excellent communication and interpersonal skills, with sensitivity to cultural and diverse participants

Creative and solution focused thinking

Strong IT skills would be beneficial
