Neurological Liaison Nurse - Kilkenny - South Australia

This is a permanent part‑time position 67.5 hours per fortnight based in Kilkenny, South Australia. Successful candidates must also be able to travel to Perth occasionally for training.
Role Overview
In this role, you’ll be an integral part of our nursing team, managing a diverse caseload of clients who need both low and high‑care support. You’ll conduct comprehensive assessments, create care plans, and provide a range of nursing services, including continence management, wound care, palliative care, and catheter care. You’ll also facilitate referrals to other health services and work closely with MSWA’s Allied Health teams to ensure the highest level of care for our clients.
Key Responsibilities
Conduct nursing assessments and develop individualised care plans.
Provide direct nursing services and clinical care.
Facilitate referrals to other necessary health services.
Collaborate with MSWA’s Allied Health teams.
Travel to clients’ homes within the community (mileage reimbursement for personal vehicle will be provided).
What We’re Looking For
AHPRA Registered
Current First Aid and CPR Certificate.
NDIS Worker Screening Check and NDIS Orientation Module (or willingness to obtain).
Full C Class driver’s licence with no restrictions.
Working with Children Check.
Nice to haves
Community nursing experience.
Background in neurological conditions or rehabilitation nursing.
Knowledge of Disability Services Legislation and NDIS.
Experience in palliative care, continence, and wound management.
Strong problem‑solving skills and computer literacy.
