About the Role As our Neurological Liaison Nurse, you’ll deliver high‑quality, person‑centred nursing care to clients living with a range of neurological conditions — including MS, Parkinson’s, Huntington’s, MND, ABI and Stroke. Working autonomously but supported by a collaborative team, you'll manage a diverse caseload and provide clinical services in clients’ homes, empowering them to remain independent and live well within their community.

Key Responsibilities

Complete holistic nursing assessments and develop individualised care plans

Provide direct clinical services (including continence, wound care, palliative support and catheter‑care)

Work closely with MSWA’s Allied Health teams to coordinate care

Facilitate timely referrals to internal and external health services

Travel throughout the community to support clients (pool vehicles available)

Must‑Haves

AHPRA registration as a Registered Nurse

Current First Aid and CPR certification

NDIS Worker Screening Check (or willingness to obtain)

Current C Class driver’s licence

Working with Children Check

Nice‑to‑Haves

Experience in community or rehabilitation nursing

Exposure to neurological conditions

Knowledge of Disability Services Legislation and the NDIS

Skills in wound care, continence, and palliative nursing

Strong clinical judgement, problem‑solving skills and computer literacy
