About the role:

As a Care Partner in our Support at Home Program, you will help older people live independently, stay connected to their communities, work collaboratively to co‑design care that reflects their goals and choices, empowering customers to live life on their terms.

We currently have exciting opportunities for enthusiastic and motivated Care Partners to join our metropolitan Support at Home teams which are based in Rostrevor, Elizabeth South and Christie Downs, for people who want to make a positive difference and support Good Lives for Older People.
What you will do:
Collaborate with a dynamic multidisciplinary team to provide comprehensive support to older individuals
In consultation with our customers, coordinate their Support at Home Program
Conduct reviews, co‑design and update care plans identifying clinical and care needs.
Build effective relationships with external stakeholders, including carers, medical professionals, and community services
Ensure robust budget management to support customers as their needs change
Provide practical and personal support in line with individual care plans
Promote independence, choice, and wellbeing
Observe, document, and report changes in a customer’s condition or needs
Deliver care in accordance with organisational policies, safety standards, and quality requirements

Manage a balanced case load, enabling you to deliver person‑centred care and a high quality customer experience.
To succeed in this role, you will need:
Certificate IV in Community Service/Ageing or similar equivalent qualification.
Experience in the aged and/or disability sector, preferably in a Care Coordinator/Case Manager role.
Ability to address individual requirements, including those with complex needs.
Intermediate computer literacy and data entry skills.
Excellent communication skills, both verbal and written.
A problem‑solving approach and a genuine desire to support older people in living a Good Life.
Ability to work independently and manage time effectively
A Police Check for working in Aged Care less than 12 months old.
