Overview
We are looking for a compassionate and proactive Care Partner to join our Right at Home Perth Northern Suburbs team, based from our Wangara office.
In this rewarding full‑time role, you'll support older Australians and individuals living with disability to live safely, independently, and meaningfully at home.
You'll work closely with clients, their families, and healthcare professionals to design and coordinate care plans that reflect each person's unique goals, values, and preferences.
You'll manage your own caseload, ensuring that services are coordinated, person‑centred, and responsive.

Qualifications
Certificate III / IV in Aged Care, Community Services, or a relevant qualification in Nursing, Allied Health, or Social Work.
Experience in care coordination, case management, or service delivery within aged care, disability, or community care.
Current Australian driver's licence and reliable vehicle.
Current First Aid and CPR certification.
National Police Check and Working With Children Check (or willingness to obtain).
Commitment to ongoing learning and person‑centred practice.
