Restorative Care Partner

to join our Community and Home Support team. Reporting to the Clinical Manager, you will: Deliver high-quality, person-centred care that promotes independence, dignity, and quality of life for older adults. Conduct comprehensive assessments and develop individualised care plans grounded in restorative, reablement, and wellness principles. Coordinate Allied Health and wellness services, including multidisciplinary team meetings, to ensure integrated care. Advocate for clients and negotiate with internal and external stakeholders to achieve optimal outcomes. Track and evaluate client progress, adjusting care plans and supports to meet changing needs. Contribute to program development, quality improvement initiatives, and a culture of continuous learning. Work across the Perth metropolitan area, providing care in clients’ homes as required. About you You are an experienced and client-focused professional with approximately three years of hands-on experience in case management or client assessments. You have a strong understanding of reablement, restorative, and wellness models, and are confident in applying these frameworks to support positive client outcomes. In addition, you will also possess: A qualification as a Registered Nurse, or an Allied Health degree with AHPRA registration Commitment to person-centred, inclusive, and evidence-based care aligned with Consumer Directed Care. Experience managing budgets, documentation, and compliance requirements. Strong stakeholder engagement, communication, and problem-solving skills. Ability to work independently and collaboratively in a multidisciplinary team.
