Access to NFP salary packaging to increase your take home pay Ozcare has a full‑time Care Partner position available at our Atherton Community Care branch. We are looking for a compassionate individual who can help support our older generation in living independently. Your Role

You’ll be responsible for planning, managing and leading client care and support across a range of programs. You’ll also play a key role in coordinating the involvement of Ozcare’s multi‑disciplinary teams and other services to support effective care. As a Care Partner you will:

Manage and coordinate care services to ensure the highest standards of care Develop and implement care plans tailored to individual client needs Liaise with clients, families, and healthcare professionals to ensure comprehensive care Foster strong relationships with clients, stakeholders, and team members, while assisting clients in meeting their unique needs and goals Ensure compliance with all relevant legislation and organisational policies Monitor and evaluate care services to continuously improve quality and efficiency Monitor the budget of Home Care Packages and ensure clients understand their funding and fees How You’ll Benefit

Demonstrate experience in case management and client care coordination in a Community Aged Care setting Have demonstrated strong verbal, written and interpersonal communication skills to communicate and work effectively with a broad range of people Be vaccinated against Influenza & at least 1 dose of Covid‑19 vaccine Provide proof of immunity, or willingness to become vaccinated against, Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox) & Pertussis (Whooping Cough) Hold a current Police Screening Certificate Provide a copy of your current First Aid and CPR Certificate Hold a current NDIS Worker Screening Check (Yellow Card) and Blue Card (Working with Children Check) Hold a current Queensland driver’s licence Completed tertiary qualifications and/or relevant experience in a health‑related field (e.g. Health and Human services) Welcoming All
