Role Description This is a full‑time role for a Care Partner based in Melbourne, VIC, with flexibility to travel for client visits in their location. In this role, you will provide high‑quality care management to older adults by overseeing daily support activities, building strong relationships with clients and their families, and assisting in the management of health and wellness plans. You will also ensure that all services are delivered with compassion and professionalism, making a positive impact on clients’ well‑being.

Responsibilities Provide day‑to‑day care management for older adults, including support activities, client‑family communication, and health plan coordination.

Build strong relationships with clients and their families to understand and meet individual needs.

Maintain professionalism and compassion in all client interactions, ensuring high‑quality service delivery.

Qualifications

Strong interpersonal and communication skills to effectively engage with clients, families, and the care team

Experience or knowledge in care management

Demonstrated customer service skills with a focus on empathy and understanding client needs

Ability to work collaboratively in a healthcare and home care setting while maintaining professionalism

Previous experience in aged care or medical care roles is an advantage

Relevant certifications or qualifications in healthcare, aged care, or a related field are preferred
