Care Partner

to join our team and support a growing number of clients in the Altona area. This role is ideal for someone who is community‑minded, culturally sensitive, and committed to providing high‑quality, person‑centred care. You’ll be working with clients across both Aged Care and Disability sectors. This is a Part Time role; 30 hours per week working from 9am to 3pm (6hrs, 5 days week) and it is a hybrid opportunity; working from home and in the community.

Key Responsibilities

Manage a portfolio of clients, ensuring their wellbeing, safety, independence and personal goals are at the centre of all decisions

Conduct comprehensive assessments and develop individualised Care Plans aligned to needs, services and budgets

Ensure services are delivered in line with Aged Care Standards, program guidelines and Home Care policy

Maintain accurate documentation, reporting and financial records for client accounts

Build and maintain positive relationships with clients, families, health professionals and external partners

Advocate for clients and ensure their cultural, linguistic, and health needs are met

Support clients to access appropriate services and resources

Liaise with families, support workers, and health professionals Enroll and manage Home Care Packages for Aged Care clients

What We’re Looking For

Experience in Aged care, disability, or community health settings

Strong communication, problem‑solving, and organisational skills

Empathy, cultural awareness, and a passion for helping others

Current First Aid & CPR, and NDIS Worker Screening check

Driver’s Licence and reliable vehicle

Full working rights in Australia

Ability to assist the growth of the business by sourcing and signing up clients
