This is a permanent full‑time role, with Monday to Friday hours
What you’ll be doing
As a Care Partner, you’ll be the go‑to person helping customers live the life they choose, with dignity, comfort, and connection. You’ll assist with developing personalised support plans, check in regularly, and make sure their care evolves with their needs. Whether it’s coordinating services, responding to urgent situations, or simply being a familiar face they trust, you’ll bring warmth, clarity, and real‑life support to every interaction.
You’ll also collaborate with clinical teams, community organisations, and other professionals to make sure each customer gets the right help at the right time. Behind the scenes, you’ll keep documentation sharp, budgets on track, and contribute to a culture of continuous improvement all while staying grounded in Suncare’s values of empathy, respect, and genuine care.
This role is offered on a full‑time permanent basis (Monday to Friday 8.30am – 4.36pm).
You’ll be joining a dedicated and supportive team servicing the Sunshine Coast region, contributing to Suncare’s growth and excellence in aged care. We offer hybrid work arrangements, including time at our Maroochydore office and working from home.
You’ll thrive in this role if you
A qualification in social, human, or community services, like a Diploma of Community Services or Certificate IV in Aged Care, or hands‑on experience that shows you know how to support people with care and dignity.
Experience creating personalised support plans, coordinating referrals, and managing care that fits people’s lives.
Clear communication skills that help you navigate tough conversations with empathy and clarity.
Strong organisational skills and the confidence to work independently while staying connected to your team.
A track record of helping people achieve meaningful outcomes, especially when working across diverse services and communities.
Knowledge of local support services and ideally, experience with Home Care Packages.
