As an experienced Clinical Lead, you will play a key leadership role in the Support at Home Program, driving high-quality, safe care for participants with complex needs. This is a senior position for a seasoned nurse ready to inspire a team, shape clinical practice and make a real impact in home-based aged care.

The Clinical Lead provides clinical oversight, mentorship and support to Clinical Care Partners, promoting excellence in care and ensuring full compliance with the Aged Care Quality Standards. You will lead by example, balancing hands‑on practice contributing to continuous improvement across the program. The role works closely and collaboratively with the Aged Care Services Team Leader to ensure aligned service delivery, coordinated care outcomes and a strong integrated approach to supporting participants.

The role maintains a clinical caseload of participants with complex needs, with responsibilities allocated approximately 20% to Clinical Lead functions, 40% to direct Nursing practice and 40% to Care Management. You will also lead clinical governance initiatives, including developing and refining clinical procedures, driving quality improvement projects and supporting investigations into clinical incidents

Desired Skills and Experience To be considered for this rewarding role you will have:

Nursing registration – RN Dv 1

Membership in AHPRA

Minimum 5 years demonstrated experience delivering services in an aged care/community setting

Demonstrated experience in clinical practice improvement, quality systems, or similar roles in aged care

Experience designing and delivering staff training and mentoring in clinical care.

Excellent interpersonal and communication skills

High level problem solving, exceptional customer service skills and analytical skills

Successful candidates will be required to undergo a National Police Check, International Police Check (if applicable), hold a valid WWCC.

Level 2 First Aid Certificate
